We check the slab, the apron at the street, and any alley approach, plus how water crosses the drive on level ground. On a tear-out we work out whether the old pour failed from the clay, a thin base, or winter scaling, because each cause changes the rebuild.
We dig to depth, seat on or key into the caliche, condition the clay so it is not dust-dry, and compact a crushed-caliche base. Poured straight onto grade, a Lubbock driveway rides the shrink-swell clay up and down until the panels fault; the base is what stops that.
Rebar sized to the vehicle weight and tied on chairs, with a thickened edge where the tires track. The grid carries the cars and bridges the seasonal soil travel, so the panels shift together rather than drifting an inch out of plane the way an unreinforced drive does.
We place an air-entrained mix rated for freeze-thaw so winter does not flake the surface, then lay expansion and control joints on a plan that ties the drive into the apron cleanly and lets the movement travel in straight lines.

That flaking is surface scaling, and it comes from water freezing and thawing inside concrete that was never air-entrained, often pushed along by de-icing salt. On the rebuild we place an air-entrained mix so the surface has room to take the freeze-thaw, and we will tell you to go light on the de-icer the handful of weeks a year it snows.
